A copy of the last three CPA fiscal year-end financial statements. 2. A copy of the most personal financial statement (s) on all major stockholders, partners, etc. 3. Contractors Background Information (Conback1,2,3.pdf), or if information is available on another firm. 4. Our Work in Process form (Workinpr.pdf). 5. Other information such as Bank, Owner, G.C. & Supplier Reference Letters, Key Personnel Resumes, Brochures and Certificate of Insurance. 6. If requesting a bid bond use form (bidbnd.pdf). |
